Dr Falak Naz’s contract to provide GP services at Burley Surgery will shortly come to an end.

This means that Dr Naz’s practice will close at 6.30 pm on Friday 29th June 2018.

NHS Calderdale Clinical Commissioning Group is responsible for the commissioning of GP services in Calderdale and it is their responsibility to ensure that patients continue to have access to a full range of GP services after the practice closes.

All patients currently registered at Burley Street will be notified of the changes by letter and advised that they should choose a new local GP practice and register before Friday 8th June 2018.

Patients that have not registered with a new GP practice by 8th June will be allocated a new GP practice and be informed of this allocation by letter.

There are also two drop-in sessions where staff will be able to answer any questions that patients may have about this change and support them to find a new GP Practice.

These drop-in support sessions will be held at: Bertie’s, Brook Street, Elland HX5 9AW on the following dates:
– Wednesday 6th June 2018 at 6pm to 8pm

– Friday 15th June 2018 at 10am to 12 noon
